
Ambulette service is available to patients confined to a wheelchair. Normal wheelchair transports are to and from doctor's offices, dialysis centers or hospitals.
Our staff and vehicles meet or exceed all state requirements for this level of service. All vehicles are equipped with safe, secure wheelchair tie downs. Safety is a primary concern for all of us.
Primary billing is through the Ohio Department of Human Services (Medicaid). Most private insurance companies and Medicare do not cover wheelchair van services. Our documentation must describe what caused the patient to be wheelchair confined, and why the patient is being transported (i.e., what the doctor's appointment is for, what procedure is being performed, etc.)
Upon our arrival at your facility please have our wheelchair van forms filled out and signed. The form may be signed by an MD, DO, RN, APN or LSW, and the signature must contain those professional initials. This form must also note the name and provider number of the attending practitioner who is ordering the transport. Please also include a patient demographics sheet ("face sheet").
